Our Procedure

In the event that your dog requires medical care while in our care, we will provide care as follows:

1. Acute Life Threatening Emergency (for example, an anaphylactic reaction)

We will immediately take you dog to PETS Referral Center emergency clinic on University Avenue in Berkeley. It is possible we will not be able to contact you prior to taking your dog to the vet.

Once the dog has been seen and is in stable condition, we will contact you and give you a full update with details and vet recommendations as necessary.

2. Non-Life-Threatening Emergency (for example, an open wound)

We will contact you and give you full details of the situation before departing for the vet. Depending on your decisions, we may use one of the following medical providers:

• Dr. David Grant (mobile vet)
• Grand Lake
• PETS Referral Center

If it is possible, we may be able to take your dog to their regular vet, however we cannot guarantee that option will be available.

If we are unable to contact you, or any of your emergency contacts in a timely manner, we will take your dog to the vet and follow their recommendations.

3. Non-Emergency Illness or Injury (for example, excessive GI upset)

We will contact you to determine the best course of action. If we are unable to contact you, or any of your emergency contacts, and we feel medical intervention is required, we will take your dog to the vet and follow their recommendations.

Consent to Medical Treatment/Procedure

If my dog becomes ill or injured, or in the event treatment is needed or advisable, as determined in the sole discretion of Every Dog Has Its Day Care®, Inc. and its officers, employees and agents (collectively, "Every Dog"), to preserve the good health and well-being of my dog, then Every Dog shall immediately attempt to notify me by telephone. If I cannot be reached by telephone or if I fail to instruct Every Dog regarding measures to be taken, or if an emergency situation exists as determined in Every Dog's sole discretion, Every Dog is authorized to engage the services of a veterinarian as outlined in this Permission to Treat form. If Every Dog is required to transport my dog to a veterinarian or medical facility to receive treatment, I, and my agents, personal representatives, assigns, and heirs, waive any rights, claims and actions against Every Dog and release Every Dog from all liabilities related to the transportation of my dog.

I understand that the veterinarian and veterinary technicians under the veterinarian’s direction may consider it necessary and advisable to perform immediate operations, treatment and procedures on my dog. I authorize Every Dog to approve such operations, treatment and procedures as recommended by the veterinarian and I, and my agents, personal representatives, assigns, and heirs, waive any rights, claims and actions against Every Dog, and release Every Dog from all liabilities related to such operations, treatment and procedures. I further agree to reimburse Every Dog for any and all expenses incurred for the transportation and medical treatment of my dog. Payment will be made upon receipt of a medical statement.

Finally, in any instance where a third party asserts any rights, claims or actions against Every Dog related to the transportation or treatment of my dog as authorized under this form, I, and my agents, personal representatives, assigns, and heirs, agree to indemnify, hold harmless and defend Every Dog from any such rights, claims, and actions (including paying reasonable attorneys’ fees, costs and expenses).

I acknowledge that I have either consulted with legal counsel or have been given an opportunity to do so before signing this Permission to Treat form and have read, understood, and voluntarily agree to execute this Permission to Treat form. This authorization shall be effective during all times that my dog is in the care of Every Dog.
